Course Description
This case-based lecture presents the multidisciplinary management of a patient with severe, refractory migraine and comorbid myofascial temporomandibular disorder, including trigger point injection, CGRP antagonist therapy, sphenopalatine ganglion block, and inpatient IV lidocaine infusion. The presentation reviews the shared trigeminal pathophysiology linking TMD and migraine, current evidence for each treatment modality, and a stepped-care framework for coordinating care in multidisciplinary approach.
Educational Objectives
1. Recognize clinical features and red flags that distinguish comorbid migraine-TMD from isolated TMD, and identify when multidisciplinary referral is warranted.
2. Describe the rationale and evidence base for interventional and pharmacologic options used in refractory craniofacial pain.
3. Formulate a stepped-care, multidisciplinary treatment plan — including triggers for inpatient escalation and outcome measures beyond pain score alone — for refractory comorbid TMD-migraine.
